CAASTLC’s case management services help people become more self-sufficient and move from a place of dependence to independence. Our case managers play a motivational and supportive role connecting people with appropriate resources while also being an advocate if needed.
Areas that the Case Manager will explore with the participant include employment and education, budgeting, nutrition, household management, parenting, personal growth and development, literacy guidance, and referrals to other community resources as needed in order to move the client towards self-sufficiency. In order that the client continue to focus on employment and/or educational goals, crisis intervention assistance is provided as needed which may include energy assistance, food, housing assistance, or family counseling assistance.
FAMILY INTAKE, ASSESSMENT AND REFERRAL
Interviews with CAASTLC staff help people evaluate how we can assist them. We schedule these interviews by appointment so please be sure to call us ahead at (314) 863-0015.
Required documentation:
- Social security documentation for all household members over one (1) year of age.
- Photo identification for the head of the household.
- Proof of residency (must be living in St. Louis County).
Family Case Management Income Eligibility Requirements – Household income may not exceed 125% of the federal poverty guidelines.
